N331 Evidence Based Practice in Nursing

Evidence Base Practice Plan Paper Guidelines

Purpose: Learn about evidence base practice process and be able to apply concepts learned into a scholarly project plan.

Directions: You will write a short paper no more than 6-7 pages (title and references not included). The paper should include all areas from the guidelines below. Be sure and include a copy of each article used.

Guidelines:

1. Write a clinical question using the PICOT format (population/patient, intervention, comparison, outcome(s), and time.

a. Should reflect uncertainty in clinical practice

b. The question is phrased using the PICOT (i.e., Patient population, Intervention, Comparison or comparison group, Outcome(s), Time period [if applicable to the question/concern]) format. The following is an example of a PICOT question:

In newborn infants in the neonatal intensive care unit (NICU), does oral sucrose administration provide better non-pharmacologic pain management than non-nutritive sucking during heel sticks?

2. Collection of the strongest, most relevant evidence to answer the question. Search the evidence and define the level of evidence found (please use the hierarchy on page 23, Polit & Beck, 2018). Select three (3) research articles that address your PICOT question.

These must be reports of single, original research studies and can be either quantitative or qualitative studies.

Do not use reviews of literature, clinical practice guidelines, or meta-syntheses. You may use mixed-methods studies if they report on a single project.

You will be marked down if your articles do not meet these criteria. You may use articles that you have selected for previous assignments (i.e. discussion forums).

3. Appraise the evidence for evidence based practice (pages 32-33, Polit & Beck, 2018). Use critical appraisal guide.

a. What is the quality of the evidence? How rigorous and reliable is it?

b. What is the evidence-magnitude of effects?

c. What is precision of estimates?

d. What evidence is there of side effects/benefits?

e. What is financial cost of applying the evidence?

f. Is the evidence relevant to my practice?

4. Extract the data. Complete the evaluation table for your two articles chosen.

5. Develop a plan for implementation

Critical Appraisal Guide

Critical Appraisal Guide for Quantitative Studies

1. Why was the study done?

• Was there a clear explanation of the purpose of the study and, if so, what was it?

2. What is the sample size?

• Were there enough people in the study to establish that the findings did not occur by chance?

3. Are the instruments of the major variables valid and reliable?

• How were variables defined? Were the instruments designed to measure a concept valid (did they measure what the researchers said they measured)? Were they reliable (did they measure a concept the same way every time they were used)?

4. How were the data analyzed?

• What statistics were used to determine if the purpose of the study was achieved?

5. Were there any untoward events during the study?

• Did people leave the study and, if so, was there something special about them?

6. How do the results fit with previous research in the area?

• Did the researchers base their work on a thorough literature review?

7. What does this research mean for clinical practice?

• Is the study purpose an important clinical issue?

Explain whether clinical practice guidelines exist for this disorder, and if so, use them to justify your recommendations. If not, explain what information you would need to take into consideration.

Discussion: Prescribing for Older Adults and Pregnant Women

In this Discussion, you will investigate a specific disorder and determine potential appropriate treatments for when it occurs in an older adult or pregnant woman.

To Prepare:

Choose one of the two following specific populations: either pregnant women or older adults. Then, select a specific disorder from the DSM-5 to use.

Use the Walden Library to research evidence-based treatments for your selected disorder in your selected population (either older adults or pregnant women). You will need to recommend one FDA-approved drug, one non-FDA-approved “off-label” drug, and one nonpharmacological intervention for treating the disorder in that population.

By Day 3 of Week 9

Recommend one FDA-approved drug, one off-label drug, and one nonpharmacological intervention for treating your chosen disorder in older adults or pregnant women.

Explain the risk assessment you would use to inform your treatment decision making. What are the risks and benefits of the FDA-approved medicine? What are the risks and benefits of the off-label drug?

Explain whether clinical practice guidelines exist for this disorder, and if so, use them to justify your recommendations. If not, explain what information you would need to take into consideration.

Support your reasoning with at least three current, credible scholarly resources, one each on the FDA-approved drug, the off-label, and a nonpharmacological intervention for the disorder.

Describe the relationships between concepts and allow for an improved understand through critical thinking and analysis.

[Revision] Lesson Evaluation-Concept Map

The effective nurse educator employs a variety of formative and summative techniques during curriculum implementation to monitor student learning.

A well-developed lesson plan employs both formative and summative assessments. A nurse educator should plan to use data from these assessments to guide instruction.

Concept maps describe the relationships between concepts and allow for an improved understand through critical thinking and analysis. To understand the link between formative and summative assessment strategies, develop a concept map describing the flow of assessment strategies throughout your own lesson plan. Be creative in your design. A rationale should be included for your choice of assessments.

Use the Topic Materials and your own research, to develop your concept map. Include the following criteria to create your concept map for this assignment:

Description of one formative assessment strategy to be used in lesson plan with rationale.

Explanation of how the data from the formative assessment will guide your instruction.

Description of one summative assessment strategy to be used in the lesson plan with rationale.

Explanation of how the data from the summative assessment will guide your future lesson plan revisions.

Description of the instructor satisfaction survey to be administered to students.

Explanation of how data from the instructor satisfaction survey will guide your teaching practice.

Cite two or three peer‐reviewed/scholarly resources no more than 5 years old in your paper.

Prepare this assessment according to the guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is not required.

Describe the ethical issues the case raises.

If it were changed to meet criteria for a model case, what ethical issues would come to the forefront?

Your initial post should contain two- to three-paragraphs with three- to four-sentences per paragraph.

The post should integrate a minimum of three readings and/or other evidence-based research articles no more than three years old and use APA formatting for citations and references.

Identify one contemporary health care issue in government and discuss the a. social, b. ethical and c. economic impact of the issue related to health policy. (List the issue and then address a, b, c.)

The focus of this discussion board is for you to gain a better understanding of how social, ethical, and economic issues can affect health policy.

In this assignment, you will have the opportunity to think about a practice issue that is of concern to you and then put together a plan for how you might get this issue addressed by a local politician.

Discussion Board Items

Identify one contemporary health care issue in government and discuss the a. social, b. ethical and c. economic impact of the issue related to health policy. (List the issue and then address a, b, c.)

With the increase in the number of baby boomers reaching retirement age and holding strong political power, what are two economic issues that both the federal and state governments will need to address?

Give one example of how nurses can affect public policy in various appointed positions.

Identify a practice issue and describe the steps you would take to influence a local politician to draft a bill in favor of your concerns.

Go to Congress.gov, (Links to an external site.), click Legislation at the very top, then check the current Congress on the left margin, and look for bills that are related to your practice issue.

List the bills that are related here, include the bill number and bill name. If none, write NONE and the search terms you used to search for bills of interest to you.

If you were to need the support of one of your legislators specific to a health policy of interest to you, which one of your representatives as you identified in previous Discussion Board Module 2 would you choose as your legislative contact?

Why? Go to either your state website or back to Congress.gov and see which committees your legislator is on and/or what their policy interests are….this could give you rationale for the why in this item.

Be specific and share what committees he/she is on and/or their policy interests…not all of them but the ones that relate to your rationale.

This DB item will be useful to you as you complete your Letter to a Legislator assignment.
