Person Centres care

Examine the stages of the care planning process, including the information needed for assessment, the input required from the Multi-Disciplinary Team (MDT), the importance of monitoring and review throughout the process.

Evaluate the Care Planning process with reference to relevant theory, demonstrating the importance of a Person-Centred approach throughout. Demonstrating how service users, their families and carers might be involved.

Provide an overview of relevant legislation, policy, guidelines in relation to current best practice when planning person centred care.

A flow chart of the process of care planning must be included (This will be uploaded separately) , showing each stage and its importance, an example of a specific area of health and social care can be used.

Describe how patients are referred to the organisation.
